Overview

Supports quality advancement team/operations and the agency with incident-related events and follow-up.  To support the mission, vision and values of Heritage Christian Services, Inc.

 

Pay rate for this position:  $24.52 / hour - $25.48 / hour

 

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting

 

Responsibilities

  • Complete 147’s/150’s incident paperwork and associated notifications (during business hours). Collaborate with Director of quality advancement and incident management specialist regarding timely completion of all associated paperwork and entry into relevant internal and external databases.
  • Complete tracking of 147’s/150’s incidents and assist in quarterly, semi-annual and annual analysis of trends across categories, programs and departments.
  • Provide survey support to quality advancement surveyors, i.e., gathering information from internal and external databases for survey completion (i.e., IRMA, etc.).
  • Chair of Incident Review Committee. Responsibilities include, but are not limited to, ensuring meetings are scheduled and held within regulatory timeframes, minutes are collected and include necessary information, training of new members and board members, ensure minutes are shared with and approved by CEO, hold semi-annual trend meetings and communicate trends across categories, departments, regions, etc. and assist with strategy for addressing systemic concerns identified, etc.
    • Facilitate weekly (minimally) review of corrective action plan (CAP) submissions, providing feedback to submitters, to ensure CAP’s are thorough, complete and relevant.
  • Provide support to incident management specialist within Apps, IRMA and other databases; assist with writing care coordinator reports and letters.
  • Facilitate training/education on thorough, sufficient evidence/remediation for corrective action plans.

Qualifications

  • Experience and knowledge of survey tools, compass process preferred
  • Experience and knowledge of OPWDD regulations preferred, particularly surrounding incidents and allegations.
  • Must have strong interpersonal skills, excellent organization, verbal and written communication skills
  • Must demonstrate ability to manage multiple details/tasks
  • Ability to create and maintain a positive work environment built on respect
  • Experience using Microsoft Word, Outlook, Excel, Therap; experience/familiarity with LincWare/Doc preferred.
  • Minimum of High School diploma or high school equivalency; college experience preferred
  • Valid driver’s license that meets Heritage Christian Services motor vehicle standards; reliable transportation to and from work.
