Overview

Supports assigned programs and/or services within the agency on continuous quality improvement efforts.  To support and promote the mission, vision and values of Heritage Christian Services, Inc.

 

Pay for this position:  $30.29 / hr - $32.70 / hr

 

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting

Responsibilities

  • Trained in OPWDD site survey protocol and therefore effectively, competently and thoroughly completes agency site surveys* as assigned.
    1. Completes surveys in required timeframe
    2. Ability to recognize, convey and support programs through any immediate health and safety concerns
    3. Ability to effectively communicate findings/outcomes from surveys
  • * Will provide extra survey support to sites identified with ongoing concerns, as assigned.
  • Skilled in execution of all types of surveys (certified sites, non-cert. sites, etc.)
  • Participate in training on person-centered reviews (PCR) and will complete assigned number of PCR’s per quarter, annually.
  • Assist in training quality advancement coordinators on survey(s) completion, as assigned
  • Provide staff education and training related to agency Compass designation, self-survey process and incident reporting and abuse.
  • Compliance with agency policies and procedures and NYS OPWDD rules and regulations, including the OPWDD site protocol manual.
  • Monitoring and oversight of unusual occurrences, and unusual med situations, as assigned.
  • Responsible for trend reports, minimally semi-annually. for assigned sites relative to unusual occurrences and unusual med situations.
  • Participate in administrative supervised evacuation drills at programs, as assigned.

Qualifications

  • Four-year degree within a human service related field preferred.
  • Minimum of one-year experience supporting individuals with developmental disabilities.
  • Experience and knowledge of OPWDD regulations preferred.
  • Has successfully demonstrated a minimum of 90% competency on survey evaluations/look behinds.
  • Must be flexible to work evenings and weekends when necessary.
  • Must have strong interpersonal skills, excellent organization, written and verbal skills. 
  • Must demonstrate the ability to manage multiple details/tasks and provide effective feedback.
  • Ability to effectively present information to audiences with a variety of knowledge/skill levels.
  • Valid driver’s license that is in compliance with Heritage Christian Services’ motor vehicle record standards, and reliable transportation to and from work.
