Heritage Christian Services is seeking candidates for the Director of Customized Supports position. You will act as a resource for individuals, their families, and support staff in creating and maintaining a way of life that fosters self reliance, encourages growth and maximizes interdependence.  Acts as the Fiscal Intermediary liaison to ensure provision and payment of supports as outlined in the individual’s self directed plan.


  1. Responsible for supervision and oversight of customized supports team, community habilitation and in home respite.
  2. Support individuals and their Circle of Support/Planning Team in person centered planning and thinking.
  3. Engage in relationships with individuals, their families/Circle of Support and support staff based on mutual respect and compassion.
  4. Foster the philosophy of self direction through active listening and establishment of outcomes based on informed choice and supported decision making.
  5. Facilitate real life problem solving and conflict resolution as needed with individuals who are self directing their supports.
  6. Provide continuity of supports for individuals choosing to customize their services through excellent communication and teamwork. Act as a point person for communication between service providers.
  7. Responsible for oversight/provision of job tasks including:
    • Documentation and billing in accordance with regulation and associated ADMs.
    • Payroll and HR functions for self hire and community habilitation staff.
    • Scheduling and supervision of staff working in the Customized Residential Options.
    • Education and training for employees associated with the Customized Residential Options.
    • Monitor finances of the department and adjust as necessary to ensure good financial standing.
  8. Act as a support for the self directed team as they act as liaisons to individuals living in Customized Options, including responsibility for:
    • Reading and understanding individual expense reports; supporting the individual and COS in making budget changes as necessary.
    • Providing education regarding FI responsibilities to individuals and their families.
    • Provide support to the broker, individual and family for recruiting, interviewing and training self-hire staff.
    • Attend ISP and COS meetings as needed/requested to ensure individual plans are implemented in accordance with the individual’s choices.
  9. To build a strong relationship with counterpart at the DDRO. To provide regular communication and work in partnership with the DDRO.
  10. To work in partnership with other key departments within the Agency to help facilitate to highest level of service for those choosing customized supports from Heritage Christian. To provide regular communication and engage in routine meetings with the finance and workforce development teams and across regions.


  • Required to have experience in a human service field and at least two years of experience working with people with intellectual/developmental disabilities.
  • Minimum of a Bachelor’s degree is required: QIDP preferred.  
  • Demonstrated excellence in teaching and developing strategies for people with intellectual disabilities.
  • Demonstrated evidence of supervisory and administrative skills. 
  • A valid, New York state driver’s license that is in compliance with Heritage Christian Services’ motor vehicle record standards.
