Overview

THIS APPLICATION SHOULD ONLY BE COMPLETED BY CANDIDATES THAT HAVE BEEN PRE-SELECTED TO PROVIDE SUPPORT THROUGH CDPAP. ON THE ASSESSMENT, YOU WILL BE REQUIRED TO ENTER THE NAME OF THE PERSON THAT HAS IDENTIFIED YOU TO BE THEIR SUPPORT STAFF.

Personal Assistants will competently and safely perform the personal care tasks, home health aide tasks and skilled nursing tasks that are included on the consumer's plan of care under the instruction, supervision and direction of the consumer or the consumer's designated representative.

As a Personal Assistant providing Consumer Directed Personal Assistance Services (CDPAS), your role and responsibilities are uniquely tailored to the person who has chosen you to support them. Heritage Christian Services is acting as the fiscal intermediary for the consumer.

Responsibilities

  • Provide health related services as outlined in the plan of care.
  • Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks as outlined in the plan of care.
  • Provide consistent support based on what really matters to the consumer.
  • Assist consumers with building meaningful relationships/friendships within their community.
  • Attend and participate in all training's, in-services and staff meetings as required.
  • Maintain the privacy of the consumer's personal information in accordance with HIPAA at all times, including storage of documentation during non-working hours.
  • Punch in/out at the start/end of each shift worked using telephony system; or complete and submit documentation once a week minimally.
  • Support each consumer in advocating for the changes he/she would like to see in the support he/she utilizes. Assist those you support in conveying this information to the appropriate people within their circle of supports.

Qualifications

  • Completion of pre-employment requirements pursuant to 10 NYCRR 766.11 (c) and (d) or any successor regulation.
  • Ability to work a rotation of shifts; flexibility with family schedules is required.
  • Ability to work independently is required.
  • Physical ability to effectively implement proper lifting and transfer techniques.
  • Must demonstrate effective verbal and written communication skills.
