Overview

The Physical Therapist will support people with intellectual and/or developmental disabilities with their physical and mobility needs in order to maximize safety, quality of life, and independence in their home and community. The Physical therapist is a key member of the multidisciplinary care team, centered around the needs and desires of the individual. The Physical Therapist will support and promote the mission, vision and values of Heritage Christian Services, Inc.

 

Pay rate for this position: $60000 - $65000 / year

 

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting

Responsibilities

  1. Consult, evaluate, and provide physical therapy treatment in certified residential or day habilitation settings;
  2. Complete necessary documentation including evaluations, treatment notes, and progress notes. Maintain HIPAA compliant communication and documentation standards;
  3. Create individualized care plans as needed, outlining the method(s) in which direct support staff are to assist the person within the domains of mobility, home exercise programs, adaptive equipment, and braces/splints;
  4. Provide training and in-services for direct support professionals in the areas of mobility, home exercise programs, adaptive equipment use, lifting/body mechanics, fall safety, and injury prevention;
  5. Collaborate and communicate with the interdisciplinary care team, including the individual receiving services, guardian (if applicable), and other care team members (both within Heritage and external supports) in order to maximize the quality of care for the individual;
  6. Recommend and facilitate acquisition of necessary adaptive equipment, which includes justification letters for insurance coverage. Collaborate with agency Occupational Therapist as needed related to evaluations and equipment maintenance;
  7. Attend medical appointments with individuals to maximize the benefit of the appointment when deemed appropriate by the therapist;
  8. Provide outpatient Physical Therapy services at the Pieters Family Life Center as assigned. The outpatient caseload primarily consists of short-term restorative Physical Therapy for the following patient populations: neurologically involved, complex medical, and wheelchair mobility evaluations;
  9. Attend meetings and agency functions as deemed necessary;
  10. Provide supervision and education services to college level Physical Therapy students as approved by administration;
  11. Maintain licensure as outlined by NYS

Qualifications

  • Physical Therapy degree from an accredited university required;
  • Licensed and currently registered by NYS to practice physical therapy required;
  • A valid NYS driver’s license that follows Agency standards required;
  • Prior experience with developmental disabilities preferred;
